Tracey Driscoll, RN, MHA
Tracey’s 20 years of experience in nursing have spanned the areas of home care, hospice, and health management. Working with seniors, hearing their stories and finding ways to make their lives easier has been her passion. Tracey’s extensive experience working with seniors has given her expertise in assessing needs and abilities, putting in place appropriate support, equipment and community resources, as well as working with assisted living and nursing home communities. Her experiences volunteering as a Certified Long Term Care Ombudsman for the State of Massachusetts have reinforced Tracey's determination to improve the lives of seniors by providing expertise and support through Transition with Care.
