Seniors and their families can count on Transition with Care whether a loved one lives at home alone or has been settled into a new residence. We can be your eyes and ears when you can’t be there yourself through documented visits, unbiased information, and open communication.
Healthcare Advocacy services provide families with peace of mind that their loved one is receiving the quality of care they need by offering the following services:
- Evaluation of a loved one’s needs and specific recommendations for appropriate care.
- Monitoring of an elder's care through regularly scheduled and documented visits.
- Open communication with family via written reports, e-mail and telephone.
- Advocate on behalf of the family to ensure quality care for loved one in their own home, nursing home or assisted living community.
- Address problems or concerns that the senior or family has with the appropriate medical or administrative staff and follow up as needed.
